Héma-Québec collects the above personal information through this form as part of its 25th anniversary activities. This request for information is mandatory if you want to submit a testimonial for publication.
Your name, email address and telephone number will be used by Héma-Québec staff to contact you, as needed, to get clarification, validate certain information or propose rephrasing based on your testimonial. If your testimonial is selected from all of the testimonials received, Héma-Québec will only publish your first name, your relationship with the organization, your testimonial and your photo on the 25th anniversary microsite (the “Microsite”) during the 25th anniversary activities. Your information will be retained until the Microsite is removed.
Héma-Québec agrees to protect your personal information and to only make it accessible to staff whose duties require it. Your personal information will be accessible to our service provider, Locomotive, who are responsible for managing and hosting the Microsite. Your personal information will be hosted in Québec.
